Our Story

Guided Path Foundation is a 501(c)(3) nonprofit organization dedicated to helping older adults remain safe, supported, and independent in their homes. We assist eligible seniors in Butler and Hamilton Counties, Ohio, who are experiencing difficulty obtaining essential medical equipment, supplies, home safety resources, and other support.

We saw seniors struggling to obtain the medical equipment and supplies they needed to remain safe at home — people being discharged from the hospital without the resources to transition home safely, and families trying to navigate care options on their own. We knew that with the right support, guidance, and compassion, a path forward is always possible.

Today, the Guided Path Foundation continues that mission — offering medical equipment, home safety assistance, RN assessments, hospital-to-home transition support, and resources to seniors who need it most. We believe that together, we can help seniors remain safe, supported, and independent at home.

Guided Path Foundation is a 501(c)(3) nonprofit organization dedicated to helping older adults remain safe, supported, and independent in their homes.

We assist eligible seniors in Butler and Hamilton Counties, Ohio, who are experiencing difficulty obtaining essential medical equipment, supplies, home safety resources, and other support.

There is no fee to submit an application. All requests are reviewed individually, and assistance is based on eligibility, demonstrated need, and available funding and resources.

Important: Guided Path Foundation is not an emergency response service. If you are experiencing a medical emergency, call 911. Please do not submit Social Security numbers, detailed medical records, or other sensitive medical information through this general application.

Guided Path Foundation understands that financial hardship affects individuals differently. You do not have to fall below a specific poverty guideline to request assistance. We consider factors such as fixed incomes, medical expenses, housing costs, insurance limitations, and other financial obligations.
